Transition From Pediatric to Adult-Oriented Health Care for the Adolescent With Chronic Illness or Disability

Adolesc Med. 1994 Jun;5(2):241-248.

Abstract

Adolescents and young adults with chronic conditions are given fewer opportunities to be independent health care consumers than their peers, and the transition from pediatric to adult-oriented care can be particularly difficult. For many chronically ill patients, transition medical services may be appropriate. The need for such services and ways to provide them are discussed.
