Coordinated care that clearly pays off. The evidence is in: Anticoagulation clinics can keep patients out of hospitals and reduce the management burden on doctors. Using pharmacists to provide expert management of warfarin therapy, Fairview Health Services in Minneapolis, MN, reports an 81% drop in its quarterly complication rates.
