The innovation of case management has been a controversial issue since nationwide implementation in 2005. The long-term condition (LTC) agenda remains prominent within all primary and secondary care services and the importance of patient-centred care is paramount. This article outlines the development and implementation of case management within a case study of a patient with the LTC chronic obstructive pulmonary disease. Key topics such as holistic care, individualized care planning and quality of life are reviewed to give awareness of the success of case management within the care of patients with complex LTCs.
