[Is intravenous immunoglobulin effective in toxic epidermal necrolysis and Stevens-Johnson syndrome?]

Abstract

Toxic epidermal necrolysis and Stevens-Johnson syndrome are severe cutaneous adverse drug reactions. Intravenous immunoglobulin is described as a therapeutic option, however its use is still controversial. Using Epistemonikos database, which is maintained by screening over 20 databases, we identified six systematic reviews, including 39 primary studies. We combined the evidence using tables for summary of findings, following the GRADE approach, and concluded there is uncertainty about the effects of intravenous immunoglobulin because the certainty of the evidence is very low; it probably leads to important adverse effects; and has high cost. Intravenous immunoglobulin should not be used outside the context of a clinical trial, or only in cases where other treatments have failed and there are no resource constraints.

La necrólisis epidérmica tóxica y el síndrome de Stevens-Johnson son reacciones cutáneas adversas severas a medicamentos. La inmunoglobulina intravenosa se describe como una alternativa terapéutica, sin embargo, su uso es aún controversial. Utilizando la base de datos Epistemonikos, la cual es mantenida mediante búsquedas en más de 20 bases de datos, identificamos seis revisiones sistemáticas que en conjunto incluyen 39 estudios primarios. Realizamos una síntesis mediante tablas de resumen de los resultados utilizando el método GRADE y concluimos que existe incertidumbre sobre la eficacia de la inmunoglobulina intravenosa porque la certeza de la evidencia es muy baja, probablemente se asocia a efectos adversos importantes y tiene alto costo. La inmunoglobulina intravenosa no debería utilizarse fuera del contexto de un estudio clínico o utilizarse solo en casos en que otras alternativas han fracasado y no existen limitaciones de recursos.
