Despite the advances in the understanding of the underlying mechanisms of chronic low back pain, treatment remains challenging in clinical practice. In this article we are focusing on the neuropathic component of chronic low back pain. Indeed, depending on which diagnostic tools are used, 16-55% of patients suffering from chronic low back pain present a neuropathic component. We here expose the specific mechanisms of neuropathic pain, the diagnostic tools available for their detection as well as the specific treatment options.
Malgré les avancées faites dans la compréhension des mécanismes sous-jacents aux douleurs lombaires chroniques, leur prise en charge demeure un défi dans la pratique clinique. Dans cet article, nous allons nous focaliser sur la composante neuropathique de la lombalgie chronique. En effet, selon les outils diagnostiques utilisés, 16 à 55% des patients souffrant de lombalgies chroniques présentent une composante neuropathique. Nous allons en exposer les mécanismes spécifiques, les outils diagnostiques disponibles à sa mise en évidence ainsi que sa prise en charge spécifique.
