We report the case of a patient with sarcoid granulomatous optic disc edema with optociliary shunts, normal visual acuity, and negative orbit computed tomographic studies. A literature review disclosed four cases of optociliary veins associated with sarcoid optic neuropathy, all the patients having very poor vision. Optociliary shunts may not carry a poor visual prognosis in optic nerve sarcoidosis.
