[The impact of the ones sexuality and autonomy awareness on the insomnia severity in patients with anxiety neurotic disorders]

Abstract

Objective: To evaluate an impact of perception of ones sexuality and autonomy on the level of insomnia in patients with neurotic/anxiety disorders.

Material and methods: A sample included 123 (93 women and 30 men) patients with neurotic/anxiety disorders (F40, F41, F43, F45) in the age from 25 to 50 years old, anxiety was the main syndrome in the clinical structure of the disorder. Test battery included Pittsburgh Sleep Quality index for the subjective sleep quality evaluation, Insomnia Severity index for stratification of patients into groups with different insomnia level. Anxiety was evaluated with the Hamilton Anxiety Rating Scale. Personality traits, sexuality and sovereignty of psychological space were assessed with the G. Ammon Ego-structure test and the Sovereignty of the individual psychological space questionnaire. Multiple regression analysis was used to study the interrelationship between insomnia severity and psychological characteristics. Insomnia severity index was chosen as the dependent variable.

Results and conclusion: An analysis of the regression model has shown the increase in insomnia index with the simultaneous augmentation of the following scales «Physical body sovereignty», «Sovereignty of objects», «Destructive aggression», «Deficient aggression», «Constructive narcissism», and decrease of the scales «Territory sovereignty», «Deficient narcissism», «Constructive sexuality». The study results imply the importance of the use of the level of sexual functioning and autonomy as the marker of identity maturity for psychotherapeutic targets in the treatment of neurotic/anxiety disorders. The use of anxiety and insomnia as the main complaints can be explained by its societal approval and need further active evaluation by the specialist for the successful psychotherapeutic treatment.

Цель исследования: Оценка влияния восприятия собственной сексуальности и автономности пациентами с тревожными расстройствами невротического уровня на выраженность инсомнии.

Материал и методы: Выборку составили 123 пациента (93 женщины и 30 мужчин) с тревожными расстройствами невротического уровня (F40, F41, F43, F45 по МКБ-10) с нарушениями сна в возрасте от 25 до 50 лет. В клинической картине в качестве ведущих психопатологических нарушений определялся тревожный синдром. Для получения данных о субъективной оценке пациентами качества сна использовался Питтсбургский опросник для определения индекса качества сна (PSQI) с последующим выделением подгрупп с различной выраженностью инсомнии на основе показателей индекса тяжести инсомнии, выраженность тревожного синдрома оценивалась при помощи шкалы тревоги Гамильтона. Исследование личностных характеристик, сексуальности и суверенного психологического пространства проводилось с помощью Я-структурного теста Г. Аммона и опросника Суверенности психологического пространства личности (СППЛ). С применением множественного регрессионного анализа (МРА) проводилось изучение связи психологических характеристик со степенью выраженности инсомнии. В качестве зависимой переменной был взят показатель индекса тяжести инсомнии.

Результаты и заключение: При анализе представленной модели можно отметить повышение индекса инсомнии при одновременном повышении оценок по шкалам «суверенность физического тела», «суверенность вещей», «деструктивная агрессия», «дефицитарная агрессия», «нарциссизм конструктивный» и снижении по шкалам «суверенность территории», «нарциссизм дефицитарный» и «конструктивная сексуальность». Исследование демонстрирует необходимость включения аспектов сексуального функционирования и уровня автономии и зрелости идентичности в качестве мишеней для психотерапии пациентов с тревожными расстройствами невротического уровня. Предъявление тревоги и инсомнии в качестве жалоб может быть связано с их определенной социальной приемлемостью и требует от специалиста более тщательного обследования для успешной психотерапевтической работы.
