The Role of Percutaneous Coronary Revascularization in Chronic Coronary Syndromes Abstract. Coronary heart disease represents the leading cause of morbidity and mortality worldwide. Optimal management of these patients is therefore crucial and includes lifestyle changes, optimal medical therapy, and coronary revascularization. This review summarizes diagnostic and therapeutic strategies of patients with chronic coronary syndromes, focusing on the 2019 European Society of Cardiology (ESC) guidelines for the diagnosis and management of chronic coronary syndromes. In particular, the role of invasive assessment and coronary revascularization in chronic coronary syndromes is discussed.
Zusammenfassung. Die koronare Herzkrankheit gehört zu den häufigsten Erkrankungen weltweit. Sie ist mit einer substanziellen Morbidität und Mortalität vergesellschaftet. In den neuen Leitlinien der ESC wird die stabile koronare Herzkrankheit neu als chronisches Koronarsyndrom (Chronic Coronary Syndrome, CCS) bezeichnet. Lebensstilveränderungen und eine optimale medikamentöse Therapie stellen im Rahmen der Sekundärprävention die Hauptpfeiler in der Behandlung von Patienten mit koronarer Herzkrankheit dar. Der Stellenwert der koronaren Revaskularisation bei Patienten mit chronischem Koronarsyndrom wird seit Jahren intensiv diskutiert. Grundsätzlich wird sie aus prognostischer oder symptomatischer Indikation durchgeführt. Wir diskutieren Aspekte der koronaren Revaskularisation bei Patienten mit chronischem Koronarsyndrom basierend auf den aktuellen Leitlinien, wobei ein besonderer Fokus auf der Indikationsstellung der perkutanen Revaskularisation und der Vorgehensweise bei nicht-obstruktiver koronarer Herzkrankheit liegt.
