Understanding the origin of pericardial effusion provides insights on management, risk of recurrence, and outcome. In infants, pericardial effusion can have various causes, including multisystem disease, infection, or cardiac tumors. We describe the case of a previously well 4-month-old infant presenting with a large pericardial effusion secondary to cardiac juvenile xanthogranulomatosis.
