Functional gastrointestinal disorders (FGIDs) include a category of diseases with gastrointestinal symptoms as the main manifestations, in which multiple factories may affect the gastrointestinal motility, induce visceral hypersensitivity, intestinal mucosal and immune function changes, the gut microbiota changes, and influence central nervous system processing the signals from gut. In essence, the pathophysiology of FGIDs is disorders of gut-brain interaction (DGBI). The characteristics of pathophysiology determine the relative specificity of the key symptoms of each FGIDs, overlapping and transition among FGIDs. Psychological factors affect patients' symptoms and symptom perception in the all levels of gut-brain interaction. The conceptual shift from FGIDs to DGBI is conducive for clinicians to understand the symptom features and the patients' feelings on the symptoms from the pathophysiological mechanism, to make a multicomponent diagnosis and select effective and individualized treatment for patients.
功能性胃肠病(FGIDs)涵盖了一大类以胃肠道症状为主要表现的疾病,其发病是多种因素引起的胃肠动力紊乱、内脏高敏感、肠道黏膜和免疫功能的改变、肠道菌群的改变以及中枢神经系统对胃肠道传入信号的处理功能异常,病理生理本质是肠-脑互动异常(DGBI)。病理生理的特点决定每一种疾病主要症状的相对特殊性,症状彼此重叠和转换,精神心理因素从肠-脑互动的各个层面影响FGIDs患者的症状和感受。故将FGIDs的概念向DGBI转变,更利于从病理生理机制理解疾病的症状和患者对症状感受,从多维度作出全面诊断,制定个体化的、有效的治疗方案。.
