A giant leap

Abstract

This editorial reflects on my own experience and advocacy, highlighting the emotional toll of leaving trusted pediatric teams and the need for support, preparation, and collaboration to ease the shift. It calls on health systems to work with young people and families to make this transition empowering, building confidence and resilience for life beyond pediatric care.
