Radiologic Windchill

Acad Radiol. 2025 Nov;32(11):6433-6445. doi: 10.1016/j.acra.2025.07.057. Epub 2025 Sep 24.

Abstract

The concept of "radiologic windchill" describes how external pressures-such as escalating imaging demand, rapid technological change, and persistent staffing shortages-can transform a routine radiology shift into an overwhelming experience. This review introduces the radiologic windchill metaphor to highlight the mounting stress faced by radiologists and critiques the current RVU-based reimbursement model, which fails to account for these cumulative burdens. We systematically examine key drivers of radiologic windchill, including rising case volumes, time pressure, case complexity, technical disruptions, and emotional burnout. By outlining the limitations of existing productivity metrics, we propose a shift toward more holistic and equitable models that better recognize the multifaceted contributions of radiologists. Adopting such approaches is essential for sustaining workforce well-being, improving patient care, and ensuring fair compensation in the rapidly evolving landscape of medical imaging.
