MAPEX-SEFH guide for the implementation of patient-reported outcomes and experience in outpatient hospital pharmacy

Abstract

The integration of patient-reported outcomes (PROMs) and patient-reported experience measures (PREMs) represents a key step toward value-based pharmaceutical care in outpatient hospital pharmacy settings. The MAPEX guideline for the implementation and use of PROMs and PREMs provides a practical framework for their integration into clinical practice, aligned with the Capacity-Motivation-Opportunity (CMO) model. This work synthesizes its main components from an applied perspective, combining available evidence with the strategic principles of the MAPEX project and prioritizing real-world applicability. The guideline proposes a structured and pragmatic approach to the implementation of PROMs and PREMs, linking their use to patient stratification, motivational interviewing, and longitudinal follow-up through dual care models combining face-to-face and digital interventions. It defines criteria for instrument selection, clinical interpretation of results, and the technological and organizational requirements for implementation, as well as key barriers and facilitators. In this context, PROMs and PREMs are positioned as actionable tools that support clinical decision-making, enable personalized pharmacotherapeutic follow-up, and make the impact of pharmaceutical care visible through patient-relevant outcomes. The integration of these instruments within the MAPEX-CMO framework reinforces person-centered, value-based care models and supports a more efficient, personalized, and clinically meaningful approach. Their implementation contributes to consolidating the role of hospital pharmacists in the evaluation of health outcomes and the improvement of patient experience.
